Need care occasionally?
Come to Frontier Family Health when you need it. Pay the one-time $250 enrollment/chart-review fee and then pay only for the care you receive.
Want an ongoing physician relationship?
Choose the $100/month Personal Physician Membership. You get enhanced access, continuity, priority scheduling, care coordination and a provider who knows you—not just a provider who sees you when you’re sick.
Need a house call?
Members and non-members can access house calls at the applicable visit rate.

¥ Recurring monthly payments to be set up at time of first visit in minimum of 3 month intervals.
For All New Patients There Is A One-Time Patient Enrollment & Chart Review Fee Of $250
Paid once, ever, when a patient establishes a relationship with Frontier Family Health.
This covers the administrative and clinical preparation involved in becoming a patient, including review of:
- Registration and demographic information
- Medical history
- Current medications and supplements
- Allergies
- Prior diagnoses and relevant medical records provided
- Current concerns/goals
- Other information submitted during onboarding
Housecalls that are >30-35 minutes travel time from Basin will incur a $25 travel fee in addition to other costs. This can be split between multiple individuals if I am able to schedule multiple patients in the same town on the same day. (ie if husband and wife both schedule Housecalls in Cody on Tuesday and they are back to back, it will only be $12.50 additional for each individual rather than $25 each)
